What is the mission of Bluegrass Outreach Initiative?

At Bluegrass Outreach Initiative, our mission is to strengthen the Northern Kentucky community by bridging the gap between available medical equipment and individuals in need, providing the necessary tools and resources to maintain dignity, wellness, and self-sufficiency at home.

We aim to provide medical equipment to those in need who are unable to obtain it. Also, we can help provide funding for specialty equipment for those in need. If we are unable to assist, we will use our network of resources to get our community the help they deserve.

Are you accepting donations?

Yes, we are accepting donations. We will accept new or lightly used medical equipment, including rolling walkers, rollators, bedside commodes, shower chairs, tub benches, wheelchairs, crutches, canes, etc. Please review accepted donations and reach out to us here.

Are you accepting a partnership/sponsor or volunteers?

We are seeking partnerships/sponsorships, board members and volunteers to help us achieve our mission. Please reach out to us here to speak to one of our members.

Are you a nonprofit?

We are officially approved as a tax-exempt organization, as a 501(c)(3). All donations are tax deductible. Anyone who has donated since 11/20/2025 can claim them as a deduction on your taxes. Please reach out if you need a receipt

Bluegrass Outreach Initiative is dedicated to donating medical equipment to individuals and communities in need. Advancing diversity, equity, and inclusion is fundamental to fulfilling our mission and ensuring that all people—regardless of background or circumstance—have fair access to essential medical resources that support health, dignity, and quality of life.

Bluegrass Outreach Initiative is committed to fostering an equitable and inclusive organization that reflects and respects the diverse communities we serve. We recognize that barriers to healthcare access disproportionately impact marginalized and underserved populations, including people with disabilities, older adults, low-income individuals, rural communities, and those facing systemic inequities.

  • BLUEGRASS OUTREACH INITIATIVE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT DIRECT-TO-CONSUMER WAIVER


    THIS IS A LEGALLY BINDING DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS. PLEASE READ CAREFULLY BEFORE SIGNING.


    1. PARTIES
      This Release of Liability and Assumption of Risk Agreement ("Agreement") is entered into between Bluegrass Outreach Initiative, a Kentucky non-profit corporation ("BOI"), and the undersigned recipient ("Recipient") of durable medical equipment ("Equipment").


    1. CONSIDERATION
      Recipient acknowledges that BOI is providing the Equipment described in the attached Equipment Receipt Form at no cost. Recipient's receipt of the Equipment constitutes valid and sufficient consideration for this Agreement.


    1. DUTY TO CONSULT MEDICAL PROFESSIONALS
      RECIPIENT EXPRESSLY ACKNOWLEDGES AND AGREES THAT:

      1. BOI is NOT providing medical advice or services;

      2. Recipient has a duty to consult with appropriate healthcare professionals regarding the proper use of the Equipment;

      3. Recipient will seek professional medical advice before using the Equipment; and

      4. Recipient will follow all manufacturer instructions for the Equipment.


    1. EQUIPMENT INSPECTION ACKNOWLEDGMENT
      RECIPIENT ACKNOWLEDGES AND AGREES THAT:

      1. Recipient has thoroughly inspected the Equipment prior to accepting it;

      2. Recipient finds the Equipment to be in acceptable condition for its intended use;

      3. Recipient has been shown how to properly operate the Equipment; and

      4. Recipient accepts the Equipment in its current "AS IS" condition with all existing defects and without warranty.


    1. DISCLAIMER OF WARRANTIES
      RECIPIENT EXPRESSLY ACKNOWLEDGES THAT THE EQUIPMENT IS PROVIDED "AS IS" AND "WITH ALL FAULTS." BOI EXPRESSLY DISCLAIMS ALL WARRANTIES, EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO ANY IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, OR NON-INFRINGEMENT.


    1. ASSUMPTION OF RISK AND RELEASE OF LIABILITY
      RECIPIENT EXPRESSLY AND VOLUNTARILY ASSUMES ALL RISK OF PERSONAL INJURY, DEATH, OR PROPERTY DAMAGE arising from the use of the Equipment and HEREBY RELEASES, WAIVES, DISCHARGES, AND COVENANTS NOT TO SUE BOI, its officers, directors, agents, volunteers, and employees FROM ALL LIABILITY TO RECIPIENT for any and all loss or damage, and any claims or demands on account of injury to person or property or resulting in death arising out of or related to the use of the Equipment, WHETHER CAUSED BY THE NEGLIGENCE OF BOI OR OTHERWISE.


    1. INDEMNIFICATION
      Recipient agrees to DEFEND, INDEMNIFY, AND HOLD HARMLESS BOI, its officers, directors, agents, volunteers, and employees from any and all claims, demands, losses, or damages, including attorney's fees, brought by any third party for injuries or damages arising out of or in any way connected with Recipient's use of the Equipment, including claims brought by persons who may be injured by Recipient's use of the Equipment, WHETHER CAUSED BY THE NEGLIGENCE OF BOI OR OTHERWISE.


    1. PHOTOGRAPH AND PUBLICITY RELEASE
      Recipient grants BOI permission to use Recipient's name, likeness, image, voice, and/or appearance as such may be embodied in any photos, video recordings, digital images, and the like, taken or made on behalf of BOI. Recipient agrees that BOI has complete ownership of such media, including the entire copyright, and may use them for any purpose consistent with BOI's mission. These uses include, but are not limited to, publications, advertisements, brochures, websites, and any promotional or educational materials. Recipient acknowledges that no compensation will be received for the use of such media and hereby releases BOI from any claims which arise out of or are in any way connected with such use.


    1. TERM AND TERMINATION
      All Equipment is permanently transferred to the Recipient as a donation as of the Effective Date, unless expressly stated otherwise in a separate written agreement.


    1. SEVERABILITY
      If any portion of this Agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect.


    1. GOVERNING LAW
      This Agreement shall be governed by the laws of the Commonwealth of Kentucky.  The parties agree that any dispute of this agreement will be resolved in the Kenton County Circuit Court of the Commonwealth of Kentucky.


    1. ACKNOWLEDGMENT
      BY SIGNING BELOW, RECIPIENT ACKNOWLEDGES THAT:

      1. RECIPIENT HAS READ THIS ENTIRE AGREEMENT;

      2. RECIPIENT UNDERSTANDS THAT THIS AGREEMENT INCLUDES A RELEASE OF LIABILITY, INCLUDING FOR BOI'S OWN NEGLIGENCE;

      3. RECIPIENT VOLUNTARILY AGREES TO BE BOUND BY ALL TERMS AND CONDITIONS; AND

      4. NO ORAL REPRESENTATIONS OR STATEMENTS HAVE BEEN MADE TO INDUCE RECIPIENT TO SIGN THIS AGREEMENT.
